The belt went out one hole. Nothing happened for months. Then one morning you're halfway up the stairs and you're breathing harder than you should be.

Why does a little weight gain make the stairs feel harder than it used to?
살짝 찐 체중이 왜 계단을 이렇게 힘들게 만들까요?
Usually it isn't the weight itself. It's a question we get in the room a lot — "Would a little weight really do this to my back?" Fat is not a storage bag. It's an endocrine organ, and the visceral fat around your organs is the loudest of the lot. It secretes signaling proteins called adipokines around the clock, and two of them, IL-6 and TNF-alpha, are the ones that matter for what happens next.

What does "low-grade systemic inflammation" actually mean?
"저등급 전신 염증"이라는 게 실제로 뭘까요?
It's the state where a routine blood panel comes back with CRP slightly above where it should be, and nobody says much about it. Too quiet to feel. Not too quiet to change how your tissue behaves. Adipose tissue's role as an active endocrine organ, releasing IL-6 and TNF-alpha into circulation, is well established in the physiology literature.¹ ² Start with the nerve endings: TNF-alpha lowers the firing threshold of peripheral nociceptors, the sensors that report tissue trouble.³ The stimulus doesn't change. The price of it does. What your body used to log as pressure now gets logged as pain.
건강검진에서 CRP가 살짝 높게 나왔는데 아무도 별말 안 하고 넘어가는, 딱 그 상태입니다. 느껴지기엔 너무 조용합니다. 그런데 조직이 굴러가는 방식을 바꿔놓기엔 충분하고요. 지방조직이 활성 내분비 기관으로서 IL-6와 TNF-알파를 혈중으로 내보낸다는 건 생리학 문헌에서 이미 확인된 내용입니다.¹ ² 먼저 신경 말단부터 봅시다. TNF-알파는 말초 통각수용기의 발화 문턱을 낮춥니다.³ 조직에 문제가 생겼다고 보고하는 센서들이죠. 자극의 크기는 그대로예요. 그 자극에 매기는 값이 달라지는 겁니다. 예전엔 그냥 압박으로 기록되던 게 이제는 통증으로 올라옵니다.
Joints take it next. Even mild synovial inflammation changes what the mechanoreceptors in the joint capsule send upstream, and the joint quietly gives up the end of its range. You'd only notice it reaching behind you for a seatbelt, or turning to back the car out.
관절이 그다음입니다. 활막에 가벼운 염증만 있어도 관절낭 안의 기계수용기가 위로 올려보내는 신호가 달라지고, 관절은 가동범위의 끝을 조용히 내어줍니다. 이건 안전벨트 잡으려고 뒤로 손 뻗을 때, 차 뺄 때 몸 돌리는 순간에나 드러나요.
Where does this actually happen in the spine?
그게 실제로 척추 어디에서 벌어질까요?
Put a hand on the top of your pelvis. The line your belt follows crosses the spine at roughly L4–L5 — the segment that carries the most load and complains the earliest. Then comes the part that surprises people, and it has a name: arthrogenic muscle inhibition. When the afferent signal from a joint changes, the nervous system reflexively turns down the motor output to the muscles that guard it.⁴ The muscle isn't injured. It's been switched off on purpose, by a system that no longer trusts that joint under load.

Why does the multifidus matter so much?
다열근이 왜 이렇게 중요할까요?
Because it's usually the first casualty, and losing it starts a cruel loop. The multifidus is the deep segmental stabilizer that holds one vertebra to the next at L4–L5 and L5–S1. An inhibited multifidus atrophies, and the space it gives up fills in with fat — MRI studies show fatty infiltration around 23–26% at these exact levels in people with chronic low back pain.⁵ The tissue that started this whole thing moves into the muscle itself. When the multifidus goes quiet, the load it was carrying gets handed to the facet joints and the intervertebral foramen. The nerve root doesn't need to be pinched. Sitting in an inflamed neighborhood is enough — which is a good part of why someone can have unremarkable imaging and a leg that still won't stop talking.
제일 먼저 당하기 때문이고, 이걸 잃으면 잔인한 고리가 시작됩니다. 다열근은 L4-L5, L5-S1 같은 분절에서 척추뼈 하나를 그 아래 뼈에 붙들어두는 심부 안정화 근육이죠. 억제된 다열근은 위축되고, 그렇게 비워진 자리에 지방이 들어찹니다. 만성 허리통증 환자의 MRI에서는 바로 이 부위의 지방 침윤이 평균 23~26% 정도로 나타납니다.⁵ 이 모든 걸 시작한 그 조직이 이번엔 근육 안으로 들어오는 거예요. 다열근이 조용해지면 그 근육이 감당하던 부하가 후관절과 추간공 쪽으로 넘어갑니다. 신경뿌리는 눌려야만 아픈 게 아니에요. 염증이 도는 자리에 앉아 있는 것만으로 충분합니다. 영상에서는 별것 없다는데 다리는 계속 말을 거는 경우, 상당 부분이 여기서 설명됩니다.
And then pain does the last piece of work. It makes you move less. Less movement means more atrophy, more visceral fat, more adipokines — and the whole thing comes back around louder than it left.
마지막 일은 통증이 맡습니다. 덜 움직이게 만들죠. 덜 움직이면 근육은 더 빠지고, 내장지방은 더 늘고, 아디포카인은 더 나옵니다. 그리고 한 바퀴 돌아 처음보다 커진 채로 돌아옵니다.
How do you break the loop?

By cutting it in one place, since a running loop has no beginning left to find. First, we bring the pain down — while it's active, nothing else the body tries sticks. Then we work on the joints and muscles, restoring motion to the segments that lost it and giving the quieted muscles something specific to do. Last, we check that what's been rebuilt is holding.
돌고 있는 걸 한 군데만 끊으면 됩니다. 한 바퀴 돌기 시작하면 어디가 시작인지 찾을 수가 없거든요. 먼저 통증을 내립니다. 아픈 동안엔 뭘 해도 소용이 없으니까요. 그다음은 관절과 근육입니다. 움직임을 잃은 분절을 되찾고, 같이 꺼져버린 근육에 구체적인 일을 시킵니다. 마지막으로 되찾은 것이 자리를 잡도록 함께 확인합니다.

Frequently asked questions
자주 묻는 질문
Does losing weight alone fix this?
Usually not by itself. Once the multifidus has atrophied and the joint has quietly narrowed its range, those changes don't reverse just because the number on the scale does. The joint and muscle side needs its own attention.
체중을 줄이는 것만으로 해결될까요? — 대개는 아닙니다. 다열근이 이미 위축되고 관절이 가동범위를 내어준 상태라면, 체중계 숫자가 바뀐다고 그 변화가 저절로 되돌아오지 않습니다. 관절과 근육 쪽은 따로 다뤄야 합니다.
Is this the same thing as sciatica?
Not exactly, though they can overlap. Sciatica usually implies nerve root compression. What's described here can produce leg symptoms without compression at all, just from inflammation sitting near the nerve root.
좌골신경통과 같은 건가요? — 겹칠 수는 있지만 정확히 같지는 않습니다. 좌골신경통은 보통 신경뿌리 압박을 뜻합니다. 여기서 설명한 건 압박 없이도 신경뿌리 근처의 염증만으로 다리 증상이 나올 수 있다는 얘기예요.
Can I have this going on and still feel mostly fine?
Yes. That's the point of "low-grade." The mid-range of motion you use daily can feel completely normal while the end range — the seatbelt reach, the backing-out-of-the-driveway twist — has already quietly narrowed.
증상 없이 이게 진행될 수도 있나요? — 네. "저등급"이라는 말이 그런 뜻이거든요. 평소 쓰는 중간 범위는 멀쩡하게 느껴지면서 안전벨트 잡을 때, 차 뺄 때 도는 동작 같은 끝범위만 이미 조용히 줄어 있을 수 있습니다.
Do I need bloodwork or imaging before coming in?
No, but if you've had any recently, bringing it along helps. A CRP slightly above range on a routine panel is often the first clue people mention.
방문 전에 혈액검사나 영상이 필요한가요? — 아니요. 다만 최근에 받으신 게 있으면 같이 가져오시면 도움이 됩니다. 건강검진에서 CRP가 살짝 높게 나온 게 첫 단서였다는 분들이 많습니다.
Is this only about age or being overweight?
No. Arthrogenic muscle inhibition and multifidus atrophy can follow any joint irritation — an old injury, a pregnancy, a long stretch of sitting — regardless of weight or age.
나이나 체중이 있는 분들만 해당되나요? — 아닙니다. 관절성 근육 억제와 다열근 위축은 관절에 자극이 쌓이면 나이나 체중과 상관없이 옵니다. 예전 부상, 임신, 오래 앉아 지낸 시기도 원인이 될 수 있어요.
