Someone has probably told you to take a deep breath. And you probably lifted your shoulders and filled your chest. That's the version most of us learned. It's also the version that leaves the main breathing muscle mostly out of the job.
Your diaphragm is a dome of muscle sitting under your lungs, separating your chest from your belly. When it does its job, it pulls downward. Your lower ribs widen out to the sides, your belly moves out a little, and your shoulders stay where they are.
Where is the diaphragm actually attached?
The diaphragm attaches at the back of your breastbone in front, around the inside of your lower ribs at the sides, and by two long tendons onto the front of your spine at T8 through T12. Those spinal attachments are the lowest and the strongest ones it has.
Most people picture it floating in the middle of the body, like a shelf. It isn't. The diaphragm doesn't sit in your body's centre. It hangs off your spine.
Why does that matter?
A muscle pulls both ends toward the middle. For the dome to travel downward, the end it hangs from has to hold still or lengthen the other way. That end is your spine.
As the lungs fill, your spine lengthens a little, the curves flatten slightly, and your ribs settle back. That's the counterweight to a chest expanding forward. Without it, every breath would tip you.
Now hold a slouch and try it. The spine can't lengthen from there, so the anchor gives way instead, and the pull has to go somewhere. It goes into the low back, the neck, the hips, the knees. Twelve to twenty thousand times a day, those joints do a little of the work your breathing was supposed to do.
That's why breathing and posture are the same conversation. Not because sitting up straight looks better, but because the muscle needs something solid to hang from.
What passes through the diaphragm?
Three structures pass through it: the vein returning blood from your legs at T8, the food pipe at T10, and the aorta at T12. The word for each opening is a hiatus, which just means a gap. Each is built differently, because each has a different job.
To find those levels on yourself: T8 is the eighth vertebra down from the base of your neck, sitting roughly behind the bottom of your breastbone. T12 is the twelfth, at the back, where your lowest ribs finish. Put one hand on each. The muscle's anchor lives between your hands.
T8 · the caval opening ·
The big vein returning blood from your legs passes here, through the tough tendon at the centre of the dome. That patch doesn't contract, so the opening widens as you breathe in and the vein widens with it. Every full breath gives the blood coming up from your legs a small lift toward the heart. Breathe shallow all day and that lift mostly doesn't happen. The blood still gets back, but your calves and your vein valves carry what the breath used to help carry. Legs that feel heavy at the end of a day you spent sitting.
T10 · the esophageal hiatus ·
The food pipe passes here, and the muscle wraps around it and grips from the outside. Your stomach already has a valve up there; this is a second one, made of the breathing muscle. The vagus nerve rides through the same gap, running from your brainstem down to your gut, and it's the main line of the system that slows your heart, moves digestion along, and lets a braced body settle.
So one gap carries both the seal and the wiring, and both answer to how you breathe. Ribs folded, diaphragm working from a shortened position all day, and the grip on the food pipe gets sloppier. Shallow chest breathing hour after hour, and the nervous system stays on the braced setting — digestion is one of the first things that setting turns down. Heartburn after meals, a full feeling that lingers, a stomach that acts up in the weeks you're stressed.
T12 · the aortic hiatus ·
The aorta slips under a stiff band of tissue here, so it never gets squeezed at all. Right beside it runs the duct that carries lymph fluid back up out of your abdomen, and that one is soft. Lymph has no pump of its own; it moves when something presses it, and the rhythm of a full breath is one of the things that presses it, all day, without you noticing. This gap also sits where the ribs finish and the diaphragm's tendons meet the spine, so it's the level that feels a stiff low back first. A waistline that feels puffy by evening, and a low back that tightens up on the days you never got a full breath in.
None of this is dramatic on any single breath. But you take somewhere between twelve and twenty thousand of them a day, and a diaphragm that only moves the top of its range does the smaller version of all three jobs, every one of those times.
Why does a stiff mid-back affect breathing?
Because the diaphragm needs the wall around it to give. Hold an empty can and squeeze it — the wall gives evenly, all the way around. Now press a dent into one side and squeeze again. Everything moves except that spot.
The wall of that can runs from your neck to your pelvis, and the dent can be anywhere along it. The diaphragm is the base inside it, dropping as air comes in. When the wall stops giving somewhere, the base can't come down level.
You still breathe, of course. You just do it higher up, with your neck and upper chest, hour after hour. That's often why shoulders feel tired at the end of a day you spent sitting.
How do I know if I'm breathing correctly?
Lie on your back with your knees bent. Put one hand on your chest and one on your belly. Breathe in through your nose and let the lower hand rise first. Your lower ribs should widen out sideways, not just lift up. Give it a minute or two.
A few things make this easier during the day. Loosen your waistband when you sit for long stretches, because a tight belt gives the diaphragm less room to drop into. Raise your screen to eye level, because looking down folds your ribs shut. Get up once an hour and take three slow breaths standing, and try the same three lying down before you sleep.
Why doesn't breathing practice work for some people?
Because the spot where the breath gets stuck is rarely the spot that caused it. The nerve that drives the diaphragm comes down from your neck. The upper back sets how much your ribs can open in the first place. And the pelvis holds the bottom of the container the whole system pushes against.
Any one of those can be the reason the breath won't go anywhere, while the mid-back is just where you feel it stop.
That's where chiropractic comes in. We examine the spine and pelvis joint by joint, find the ones that have stopped moving, and restore motion in them by hand. The point of the exam is to figure out where your pattern actually starts, because chasing the place that hurts rarely holds. Then the movement gets reset from that starting point, and the breathing you practice at home finally has somewhere to go.
So try it tonight. Lie on your back with your knees bent, one hand on your chest and one on your belly, and breathe in so the lower hand rises first. If the lower hand won't move, or your shoulders are still tired on the days you barely moved, come in and let's check it together, from your neck down to your pelvis. Wear something loose at the waist when you do.
Frequently asked questions
Q. Where exactly is the diaphragm attached?
At the back of your breastbone in front, around the inside of your lower ribs at the sides, and by two tendons onto the front of the spine at T8 through T12. Those spinal attachments are the lowest and strongest ones it has.
Q. What are the three openings in the diaphragm?
The caval opening at T8 for the vein returning blood from the legs, the esophageal hiatus at T10 for the food pipe and the vagus nerve, and the aortic hiatus at T12 for the aorta and the lymphatic duct beside it.
Q. How can I tell if I'm breathing with my diaphragm?
Lie on your back with your knees bent, one hand on your chest and one on your belly. Breathe in through your nose. The lower hand should rise first and your lower ribs should widen sideways, while your shoulders stay still.
Q. Can breathing affect posture?
They share the same anchor. For the diaphragm to drop, the spine it hangs from has to lengthen. In a slouch it can't, so the effort goes into the low back, neck, hips and knees instead.
Q. What does a chiropractor check for breathing problems?
We examine the spine and pelvis joint by joint to find where motion has stopped. The point is to locate where the pattern starts, since the place the breath catches is often not the place that caused it.
Q. Should I see a chiropractor or just practise on my own?
Try the two minutes on your back first. If the lower hand won't move, or your shoulders are tired on days you barely moved, that's worth examining in person.
