Hi, this is Levitas Chiropractic & Wellness in Edmonds.
You get out of the truck, or straighten up from the machine, and the first few steps are not quite yours. The hip is late. Something in the back of the leg has to be talked into it.
트럭에서 내리거나 기계 앞에서 허리를 펴고 나면 처음 몇 걸음이 어색합니다. 고관절이 한 박자 늦게 따라오고, 허벅지 뒤쪽 어딘가를 달래가면서 걷게 되죠.
Patients usually mention this at the very end of the visit, half apologising for it, saying it is probably nothing. It is often the part before an injury. It is also the part where a small amount of work still goes a long way, because there is nothing to repair yet.
이 얘기는 대개 진료가 다 끝나갈 때쯤 나옵니다. 별거 아닌데요, 하면서 꺼내시죠. 그런데 이게 뭔가 탈이 나기 전 단계인 경우가 많습니다. 아직 고칠 게 생기기 전이라, 크게 손대지 않고 관리로 넘어갈 수 있는 때거든요.
We also ask everyone whether they own a foam roller. Most say no, or say yes and then admit it has not moved in a year. When we ask what it is supposed to do, the answers run out fast.
저희는 오시는 분들께 폼롤러 있으시냐고 꼭 여쭤봅니다. 대개 없다고 하시고, 있다고 하신 분도 일 년째 그대로 있다고 하시고요. 그래서 그게 어디에 좋은 거냐고 여쭤보면 대답이 금방 끊깁니다.
What the Thing Actually Is
그게 뭐냐면
The object. A firm cylinder, usually about the length of your forearm and roughly the width of a dinner plate.
생김새. 단단한 원통입니다. 길이는 대개 팔뚝만 하고, 굵기는 접시 하나 정도 됩니다.
How it is used. You put it on the floor, lay part of your body across it, and let your own weight press down while you move slowly along the muscle.
쓰는 법. 바닥에 놓고 그 위에 몸의 한 부분을 올린 다음, 자기 체중으로 누르면서 근육을 따라 천천히 움직입니다.
Why nobody explains it. No settings, no power, nothing to learn. It looks too simple to need explaining.
왜 아무도 설명을 안 해주냐면. 설정도 없고 전원도 없고 배울 것도 없어서요. 설명이 필요해 보이지가 않습니다.
So people buy one, lie on it, find that it hurts, and put it behind the sofa. That is the usual ending, and it is not the user's fault. To know what to do with it you have to know what it is for, and that starts smaller than you would expect.
그래서 사서, 누워보고, 아프니까, 소파 뒤에 둡니다. 대개 이렇게 끝나고요. 이건 쓰신 분 잘못이 아닙니다. 어디에 쓰는 물건인지 알아야 어떻게 쓸지가 나오는데, 그 출발점이 생각보다 훨씬 작은 데 있어요.
One. A Cell Cannot Stand Up on Its Own
하나. 세포는 혼자 서 있지 못합니다
Your body is tens of trillions of cells. So why does it not slump into a puddle when you sit down.
우리 몸은 세포 수십조 개로 되어 있습니다. 그런데 왜 의자에 앉았을 때 흘러내리지 않을까요.
Because the space between them is full. That filling is called the extracellular matrix. Older textbooks treated it as packing material. It is closer to the building the cell lives in, and to the instructions it lives by.
세포 사이가 빈틈없이 채워져 있기 때문입니다. 그 채우고 있는 걸 세포외기질이라고 부릅니다. 예전 교과서는 이걸 그냥 충전재로 봤어요. 지금은 세포가 사는 집이자 세포한테 지시를 내리는 환경으로 봅니다.
Two. What Holds You Up Is Mostly Water
둘. 몸을 지탱하는 건 사실 물입니다
A rope resists pulling. It does nothing against being squashed. So what absorbs the impact when you come down off a truck step.
밧줄은 당기는 힘에는 강한데 눌리는 데는 아무 소용이 없습니다. 그럼 트럭 발판에서 내려설 때의 충격은 뭐가 받아낼까요.
The charge. GAG chains carry a strong negative charge, and that charge pulls water molecules in and traps them.
전하. GAG 사슬은 강한 음전하를 띱니다. 음전하가 물 분자를 끌어당겨서 조직 안에 가둬요.
The pressure. Trapped water makes the tissue push outward from the inside. That outward push is called turgor pressure.
압력. 갇힌 물이 조직을 안에서 밖으로 밀어냅니다. 이 미는 힘을 팽윤압이라고 부릅니다.
The clever part. When load squeezes water out, the remaining negative charges are forced closer together, like pushing two magnets the wrong way. The repulsion climbs steeply, and the pull drawing water back in climbs with it.
잘 만들어진 부분. 하중이 걸려 물이 밀려 나가면 남은 음전하끼리 거리가 가까워집니다. 같은 극 자석 두 개를 억지로 붙이는 것과 같아요. 반발력이 급격히 커지고, 물을 다시 빨아들이는 힘도 같이 커집니다.
The harder you press, the harder it pushes back. That is not a spring. That is hydraulics. And it has one requirement, which is that the water has to keep moving in and out.
누를수록 되미는 힘이 세집니다. 스프링이 아니라 유압 장치예요. 그리고 조건이 하나 있습니다. 물이 실제로 드나들어야 합니다.
ONE NUTRIENT WORTH KNOWING · 알아두면 좋은 영양소 하나
Vitamin C. Collagen is three strands wound into a rope, and what locks the twist in place is a modified form of the amino acid proline. Making that modification requires vitamin C. Without it the rope unwinds at body temperature, which is scurvy, and it is why sailors' old wounds reopened. Collagen is being rebuilt every day, and that rebuilding runs on a nutrient your body cannot make on its own or store for long. Half a red bell pepper covers a day. More than an orange, and you can eat it raw on a break.
비타민 C. 콜라겐은 세 가닥이 꼬인 밧줄인데, 그 꼬임을 잡아주는 게 프롤린이라는 아미노산의 변형된 형태입니다. 이 변형에 비타민 C가 반드시 필요해요. 없으면 밧줄이 체온에서 풀려버립니다. 그게 괴혈병이고, 옛날 선원들의 다 나은 상처가 다시 벌어지던 이유고요. 콜라겐은 매일 새로 만들어지고 있고, 그 작업이 몸에서 만들지도 오래 저장하지도 못하는 영양소 하나에 걸려 있습니다. 빨간 파프리카 반 개면 하루치가 됩니다. 오렌지보다 많고, 쉬는 시간에 그냥 썰어 먹을 수 있어요.
Three. What Eight Hours in One Position Does
셋. 여덟 시간을 한 자세로 보내면
Whether you are sitting in a cab, standing at a machine, or kneeling on a floor, holding one position all day means that tissue gets almost no cycle of load and release. The water stops going in and out. Two things follow.
운전석에 앉아 계시든 기계 앞에 서 계시든 바닥에 무릎을 꿇고 계시든, 하루 종일 한 자세로 있으면 그 조직에는 압박과 해제의 왕복이 거의 없습니다. 물이 들고 나질 않아요. 그러면 두 가지가 따라옵니다.
The layers get sticky
층 사이가 끈적해집니다
What is in there. Fascia is not one sheet, it is several. Between them sits a thin watery film that lets one layer slide over the next.
거기 뭐가 있냐면. 근막은 한 겹이 아니라 여러 겹입니다. 그 사이에 물기 많은 얇은 막이 끼어 있어서 층끼리 미끄러지게 해줘요.
What it is made of. Mostly hyaluronan, the same molecule you may know from knee injections. Think of it as the oil between the sheets.
그 막의 정체. 주성분이 히알루론산입니다. 무릎 주사로 들어보셨을 그 물질이에요. 층과 층 사이의 엔진 오일이라고 보시면 됩니다.
Why it changes. Its thickness is not fixed. Left alone the chains tangle and it thickens. Shear it or warm it and the tangles let go. Cold honey will not pour until you stir it.
왜 달라지냐면. 점도가 고정돼 있지 않아서요. 가만히 두면 사슬이 엉키면서 뻑뻑해지고, 문지르거나 온도가 오르면 그 엉킴이 풀립니다. 냉장고에서 막 꺼낸 꿀이 안 흐르다가 세게 저으면 부드러워지는 것과 같아요.
That is your morning stiffness, and it is why it eases once you get moving. It is not only muscle warming up.
아침에 뻣뻣하다가 좀 움직이면 풀리는 게 이겁니다. 근육이 데워져서만은 아니에요.
The collagen settles in
콜라겐이 자리를 잡습니다
Who builds it. Fibroblasts live in that tissue and constantly break down old matrix and lay down new.
누가 만드냐면. 조직 안에 섬유아세포가 상주하면서 낡은 기질을 분해하고 새로 만드는 일을 계속합니다.
What tells them how. The forces landing on them. No movement means no direction.
뭘 보고 만드냐면. 자기한테 도착하는 힘을 보고요. 움직임이 없으면 방향 정보가 없습니다.
What you get. New fibres go down disordered and start cross-linking to each other.
그래서 어떻게 되냐면. 새로 놓이는 섬유가 무질서하게 깔리고, 섬유끼리 가로로 붙기 시작합니다.
A cell has no eyes. It reads the world through the forces arriving at it. In tissue that has not moved in a long while, the message is short. Nothing happens here.
세포는 눈이 없습니다. 자기한테 도착하는 힘으로 바깥을 읽어요. 오래 안 움직인 조직에서 세포가 받는 메시지는 짧습니다. 여기는 아무 일도 일어나지 않는다.
Four. How That Turns Into Pain
넷. 그게 어떻게 통증이 되는가
Fascia wraps each muscle fibre, then the bundle, then the whole muscle, and it does not stop there. It carries on into the next one, from the sole of the foot up the calf, across the pelvis and up the back to the base of the skull. Think of an orange, where the film around each segment continues into the peel.
근막은 근섬유 하나하나를 싸고, 그 다발을 싸고, 근육 전체를 싸고, 거기서 안 멈추고 옆 근육으로 이어집니다. 발바닥에서 종아리로, 골반을 지나 등을 타고 뒤통수까지가 하나로 연결돼 있어요. 오렌지를 떠올리시면 쉽습니다. 알갱이를 싸는 막, 조각을 싸는 막, 껍질까지가 다 이어져 있죠.
So stiffness in one place does not stay in that place. When a joint cannot use its full range, something nearby makes up the difference. The body finds a way to move. It just uses tissue that was never meant for that job, and a few months later, that is the tissue that hurts.
그래서 한 자리가 뻑뻑해지면 그 자리에서 끝나지 않습니다. 어느 관절이 제 범위를 다 못 쓰면 그만큼을 옆이나 위아래가 대신 냅니다. 몸은 어떻게든 움직이거든요. 다만 원래 그 일을 하라고 만들어지지 않은 조직이 대신 하게 되고, 몇 달 뒤에 아파지는 게 바로 그 조직입니다.
The shape repeats. An ankle that will not bend sends the angle to the knee. A hip that will not rotate sends the turn to the low back. An upper back that will not extend makes the shoulder force the last of the reach. This is why working the sore spot goes nowhere. The stiff joint is usually one segment away from the pain.
이게 늘 같은 모양으로 나타납니다. 발목이 안 접히면 무릎이 대신 각도를 내고, 고관절이 안 돌면 요추가 대신 돌고, 등 윗부분이 안 펴지면 어깨가 팔을 억지로 올립니다. 아픈 데를 아무리 눌러도 소용없는 이유가 여기 있어요. 굳어 있는 건 보통 한 칸 옆입니다.
The place that hurts is often not the cause. It is where the cause ended up.
아픈 자리는 원인이 아니라, 원인이 밀려서 도착한 곳인 경우가 많습니다.
Five. So This Is What It Is For
다섯. 그래서 폼롤러가 하는 일
One. It thins the film between the layers
하나. 층 사이 물기를 다시 묽게 만듭니다
Slow bodyweight pressure puts shear through that watery film and raises its temperature a little. The tangles release, the viscosity drops, and the layers start sliding again. You are putting a cycle of load and release back into tissue that has had none all shift.
체중을 실어 천천히 굴리면 그 얇은 막에 전단력이 걸리고 마찰로 온도가 조금 오릅니다. 엉킴이 풀리면서 점도가 내려가고, 층끼리 다시 미끄러지기 시작해요. 근무 내내 아무 자극도 못 받은 조직에 압박과 해제의 왕복을 넣어주는 겁니다.
Two. It sends a signal to the nervous system
둘. 신경에 신호를 보냅니다
In practice this is the larger of the two, and it is the part almost nobody hears about.
실제로는 이쪽이 훨씬 큰데, 이 얘길 들으신 분은 거의 없더라고요.
Fascia is a sensory organ. It is among the most densely innervated tissue you own. It used to be treated as wrapping around muscle.
근막은 감각 기관입니다. 몸에서 신경 밀도가 가장 높은 조직 중 하나예요. 예전에는 근육 싸는 포장지로 봤고요.
Fast fibres win the race. Press it and the thick, fast sensory fibres fire together. They reach the spinal cord ahead of the pain signal, which travels on thin slow fibres. Once the fast traffic has the doorway, the pain signal cannot get through.
빠른 신경이 먼저 도착합니다. 누르면 굵고 빠른 감각신경이 한꺼번에 발화해요. 이 신호가 통증 신호보다 먼저 척수에 도착합니다. 통증은 가늘고 느린 섬유를 타고 오거든요. 빠른 신호가 먼저 자리를 차지하면 통증 신호는 위로 못 올라갑니다.
The brain re-reads the area. The signal keeps climbing to the brainstem, where the reading is that pressure is arriving but damage is not, and that verdict comes back down, lifting the pain threshold generally.
뇌가 그 부위를 다시 평가합니다. 신호는 뇌간까지 올라갑니다. 압박은 들어오는데 손상 신호는 아니다. 그 판정이 다시 내려오면서 통증 문턱이 전체적으로 올라가요.
You can check this yourself. Roll one calf, then test the ankle you never touched. That side has loosened too, and the roller never reached it.
직접 확인해보실 수 있습니다. 한쪽 종아리만 굴리고 나서 반대쪽 발목을 움직여 보세요. 그쪽도 부드러워져 있습니다. 롤러가 닿지도 않았는데요.
Six. If It Only Ever Hurt
여섯. 아프기만 했다면 이유는 이겁니다
Most people use it as a punishment. They lean in until their eyes water, because somebody said the knot was glued down and had to be broken apart.
대부분 이걸 벌처럼 씁니다. 눈물이 핑 돌 때까지 체중을 싣죠. 뭉친 자리가 들러붙어 있으니 부숴야 한다고 어디선가 들으셨을 거예요.
The numbers do not allow it. Stretching deep fascia by a single percent takes something close to nine hundred kilograms of force. A person on a foam cylinder produces a small fraction of that, and if that force ever did arrive, the tissue would tear before it lengthened.
숫자가 허락을 안 합니다. 심부근막을 1퍼센트 늘리는 데만 900킬로그램에 가까운 힘이 듭니다. 롤러 위에 체중을 실어서 낼 수 있는 힘이 아니에요. 그 힘이 실제로 걸린다면 늘어나기 전에 조직이 먼저 찢어집니다.
And force works against you. Past a certain intensity the body files the input as a threat. Sympathetic tone rises, the muscle contracts to protect itself, and the tissue under the roller gets harder rather than softer.
게다가 세게 하면 역효과입니다. 통증이 일정 수준을 넘어가면 몸이 이걸 위협으로 분류해요. 교감신경이 올라가고 근육은 자기를 보호하려고 오히려 수축합니다. 롤러 밑에서 조직이 풀리는 게 아니라 단단해지는 거죠.
Bruising is not progress. Keep going and you compress capillaries and cause small bleeds. A bruise where you rolled yesterday is damage.
멍은 좋아지는 신호가 아닙니다. 계속 누르면 모세혈관이 눌리고 미세출혈이 생겨요. 어제 굴린 자리에 멍이 올라왔다면 조직이 상한 겁니다.
So if it hurt and nothing changed, the tool was not the problem. The instruction was.
아프기만 하고 아무것도 안 바뀌었다면, 도구가 문제였던 게 아닙니다. 배운 방법이 틀렸던 거예요.
Seven. What It Cannot Do
일곱. 폼롤러가 못 하는 것들
It does not last. The film re-thickens as it cools and the neural change runs on the order of tens of minutes. Roll, then sit back down, and you have spent all of it.
오래 안 갑니다. 층 사이 물기는 식으면서 다시 끈적해지고, 신경 쪽 변화도 몇십 분 단위예요. 굴리고 나서 도로 앉으면 방금 만든 걸 전부 흘려보내는 겁니다.
It cannot reach a joint. The roller loads the muscle belly and the tissue over it. If a joint has stopped travelling through its range, the roller does nothing to that joint. If a stiff hip is why the low back hurts, rolling the thigh leaves the hip exactly where it was.
관절에는 닿지 않습니다. 롤러는 근육 바닥과 그 위 조직에 압박을 겁니다. 관절이 제 범위를 못 나가고 있다면 그 관절 자체에는 아무것도 하지 못해요. 고관절이 굳어서 허리가 아픈 거라면, 허벅지를 아무리 굴려도 고관절은 그대로입니다.
It cannot change where the work goes. How your weight lands on that leg when you walk. Whether the hip is doing its share. Where the job gets handed off.
힘이 어디로 실리는지는 바꾸지 못합니다. 걸을 때 체중이 그 다리에 어떻게 떨어지는지, 고관절이 제 몫을 하는지, 그 일이 결국 어디로 넘어가는지.
Which is why the same spot keeps coming back. If the cause is one segment away, the spot will still be there in a few days no matter how long you work it.
그래서 같은 자리가 계속 돌아옵니다. 원인이 한 칸 옆에 있으면, 증상 자리를 아무리 오래 굴려도 며칠 뒤에 그대로예요.
Eight. What Happens When You Come In
여덟. 오시면 뭘 하느냐면
The picture most people carry is a table and a loud noise. That is one small part of it, and it is not where we start.
대개 머릿속에 테이블 하나랑 뚝 소리 나는 장면이 있으실 겁니다. 그건 아주 일부고, 거기서 시작하지도 않아요.
We watch you move first. Walk, sit down, stand up, reach overhead, turn and look behind you.
먼저 움직이시는 걸 봅니다. 걷고, 앉고, 일어나고, 팔을 머리 위로 올리고, 뒤를 돌아보시는 것까지요.
Then we test each joint by hand. We take it through its range and find the one that stopped travelling. It is usually not where it hurts.
그다음 관절을 하나씩 손으로 확인합니다. 끝까지 움직여 보면서 멈춰 있는 자리를 찾아요. 대개 아픈 데가 아닙니다.
Restoring movement there does two things. The work goes back where it belongs, and the signal that joint sends up to the brain comes back online. A joint transmits force, and it also reports its own position. A joint that has not moved in a long time reports poorly, and the brain guards a region it cannot read clearly.
그 관절을 다시 움직이게 하면 두 가지가 일어납니다. 일이 원래 가야 할 자리로 돌아가고, 그 관절에서 뇌로 올라가던 신호가 정상으로 돌아옵니다. 관절은 힘을 전달하는 곳이면서 자기 위치를 뇌에 보고하는 센서이기도 하거든요. 오래 안 움직인 관절은 보고가 흐려지고, 뇌는 잘 안 읽히는 부위를 방어합니다.
And this is where the roller earns its place. Open a joint and leave the surrounding tissue as it was, and it closes again within days. The tissue has to glide in that new range, and the range has to be used. The roller is how you carry that on at home.
여기서 폼롤러가 다시 필요해집니다. 관절을 열어놓고 조직을 그대로 두면 며칠 안에 원래대로 돌아갑니다. 조직이 그 새 범위에서 미끄러져 줘야 하고, 그 범위를 실제로 써야 유지돼요. 롤러는 그걸 집에서 이어가는 도구입니다.
They are not alternatives to each other. They are an order.
둘은 경쟁하는 게 아닙니다. 순서가 있는 거예요.
Nine. If You Start Tonight
아홉. 오늘 저녁부터 해보신다면
Stay under the line. Uncomfortable, but your breathing stays normal. The moment you catch yourself holding your breath, you have crossed it and the muscle is already guarding.
선을 넘지 마세요. 불편하긴 한데 숨은 평소처럼 쉬어지는 정도. 숨을 참고 있다는 걸 알아차린 순간 이미 넘긴 겁니다. 그때는 근육이 벌써 방어하고 있어요.
Go slowly. Fast passes chop the input into fragments the nervous system cannot read. Move slowly enough to feel the resistance change under you, stop where it does, and breathe there.
천천히 가세요. 빠르게 왕복하면 자극이 잘게 끊겨서 신경이 읽을 수가 없습니다. 밑에서 저항이 달라지는 지점을 느낄 만큼 천천히 가시고, 달라지는 자리에서 멈춰 숨을 몇 번 쉬세요.
Then use it. It will not stay soft for long. Stand up and move through the range you just opened. The range opened because your body reclassified that angle as safer, not because the muscle got longer, and that holds only if you use it.
그리고 써먹으세요. 그 상태가 오래 안 갑니다. 일어나서 방금 열린 범위로 실제로 움직이세요. 가동범위가 늘어난 건 근육이 길어져서가 아니라 몸이 그 각도를 덜 위험하다고 판단하기 시작해서입니다. 실제로 써야 그 판단이 유지돼요.
Where not to put it
대지 않는 자리
The low back. No ribs down there to spread the load, so the joints take all of it. Work the glutes or the upper back instead.
허리 아래쪽. 갈비뼈가 없어서 하중을 나눠 받을 데가 없습니다. 엉덩이나 등 위쪽을 대신 굴리세요.
Over bone. The outside of the knee, the elbow, the point of the hip.
뼈가 튀어나온 자리. 무릎 바깥, 팔꿈치, 골반 옆으로 만져지는 뼈.
Anywhere that tingles or shoots. That is a nerve, not fascia. Come off it.
저리거나 찌릿한 자리. 그건 근막이 아니라 신경입니다. 즉시 피하세요.
Fresh injury, heat, or swelling. Not that day.
다친 지 얼마 안 됐거나 열이 나거나 부어 있는 곳. 그날은 하지 않습니다.

진료실에서
The roller puts activity back into tissue that has stopped receiving any. For someone who spends the day in one position, that is not a small thing, and it costs almost nothing. It also only does what it can do. A joint that has stopped moving, and weight that keeps landing in the wrong place, do not get sorted out from the floor.
폼롤러는 아무 자극도 못 받고 있던 조직에 다시 일이 일어나게 만드는 도구입니다. 하루를 한 자세로 보내는 분들에게 이건 작은 일이 아니고, 돈도 거의 안 듭니다. 그리고 롤러가 할 수 있는 데까지만 합니다. 관절이 멈춰 있는 문제, 체중이 자꾸 엉뚱한 데로 실리는 문제는 바닥에 누워서 해결되지 않아요.
Hip, low back, knee, shoulder — if the same place has been coming back for months, finding out which joint sent its work there is faster than working the sore spot harder. We find it, open it, and then set up what you do at home so it holds.
고관절이든 허리든 무릎이든 어깨든, 같은 자리가 몇 달째 돌아오고 있다면 아픈 데를 더 세게 누르는 것보다 어느 관절이 그 일을 여기로 넘겼는지 찾는 게 빠릅니다. 찾아서, 열고, 그게 유지되도록 집에서 뭘 하실지까지 정리해 드려요.
Come in the clothes you can move in.
움직이기 편한 옷으로 오시면 됩니다. 걷고 앉았다 일어나고 팔 올리시는 걸 볼 거라서, 무릎과 어깨가 자유롭게 움직이는 옷이면 충분해요.
