Three months of band rows and overhead presses, barely a day missed. You checked your form in the mirror every time. And the ache along the top of your right shoulder, the one that climbs into the base of your neck whenever you press overhead, is right where it was when you started.

Hi, this is Levitas Chiropractic & Wellness in Edmonds. We hear this almost every week: "I've done every exercise I was given. Why does my shoulder still hurt when I lift my arm?" Usually the work was real. The problem sits somewhere the mirror can't show you: the shoulder blade.
안녕하세요, 에드먼즈의 리비타스 카이로프랙틱입니다. 진료실에서 거의 매주 듣는 말이 있어요. "받은 운동 다 했는데, 왜 팔만 들면 아직도 어깨가 아프죠?" 대부분 정말 열심히 하셨습니다. 문제는 거울로는 안 보이는 곳, 날개뼈에 있어요.
The mirror shows the arm, not the shoulder blade
거울은 팔만 보여준다
A mirror shows the path of your arm. It can't show where your shoulder blade sits while the arm moves, and that's where every rotator cuff muscle starts. When the shoulder blade slides and wobbles, those muscles are pulling from a base that won't hold still. Think of throwing a ball from a rocking canoe. Same arm, shorter throw.
거울로 보이는 건 팔이 가는 길뿐입니다. 팔이 움직이는 동안 날개뼈가 어디 있는지는 안 보이죠. 그런데 어깨를 잡아주는 rotator cuff는 전부 그 날개뼈에서 시작해요. 날개뼈가 미끄러지고 흔들리면 이 근육들은 가만있지 않는 발판을 딛고 당기는 셈입니다. 흔들리는 카누 위에서 공을 던지는 것과 같아요. 팔 힘은 그대로인데 공이 멀리 안 갑니다.
This has been measured. When the shoulder blade was simply steadied by hand, the same shoulder muscle tested stronger, in injured and healthy shoulders alike. Plenty of muscles that test weak were never weak. They had nothing firm to pull from.
실제로 재 본 연구가 있습니다. 검사자가 손으로 날개뼈를 받쳐 흔들리지 않게만 했는데, 같은 어깨 근육이 더 세게 나왔어요. 다친 어깨든 멀쩡한 어깨든 마찬가지였습니다. 약하다던 근육 중 상당수는 약한 게 아니었던 거죠. 버틸 데가 없었을 뿐입니다.
It starts in the upper back
시작은 등 윗부분이다
The shoulder blade rests on the back of the ribs, so its position depends on the shape of the upper back underneath it. When the upper back rounds forward, the shoulder blade loses the surface it needs to tip back and rotate upward as the arm rises. The body holds it in place anyway, using the rhomboids and levator scapulae, which pull it back and hitch it up toward the neck. That's why your shoulders creep toward your ears when you press. It's also why the familiar cue "squeeze your shoulder blades together" feels right and makes things worse. It asks those same two muscles to work even harder.
날개뼈는 갈비뼈 뒤에 얹혀 있어서, 그 밑의 등 윗부분이 어떤 모양이냐에 따라 자리가 정해집니다. 등 윗부분이 앞으로 굽으면 날개뼈는 팔을 들 때 뒤로 젖혀지고 위로 돌아갈 바닥을 잃어요. 그래도 몸은 날개뼈를 붙잡아 두는데, 이때 쓰는 게 rhomboid와 levator scapulae입니다. 날개뼈를 뒤로 당기면서 목 쪽으로 끌어올리는 근육들이죠. 프레스할 때 어깨가 귀 쪽으로 슬금슬금 올라가는 이유가 이겁니다. 흔히 듣는 "날개뼈 꽉 모으세요"가 맞는 것 같은데 오히려 더 틀어지게 하는 이유도 같고요. 바로 이 두 근육을 더 쓰게 만들거든요.
The change doesn't stay at the shoulder blade. The shallow socket where the arm bone sits turns to face a new direction, and the upper arm rolls inward to meet it. The elbow and wrist line up behind it. When slumped and upright sitting were compared, the upper trapezius, the muscle across the top of the shoulder, worked harder in the slump, while the serratus anterior, which holds the shoulder blade flat against the ribs, did less. From the outside, it's the same press. Underneath, a different muscle is lifting the weight.
변화는 날개뼈에서 멈추지 않습니다. 날개뼈에서 팔뼈가 끼워지는 얕고 오목한 자리가 방향을 틀면, 위팔은 거기 맞춰 안으로 돌아가요. 팔꿈치와 손목도 그 뒤를 따라 줄을 서고요. 구부정하게 앉았을 때와 바로 앉았을 때를 비교한 연구에서는, 구부정할 때 어깨 위쪽을 덮는 upper trapezius가 더 일하고, 날개뼈를 갈비뼈에 붙여 주는 serratus anterior는 덜 일했습니다. 겉으로는 같은 프레스예요. 속에서는 다른 근육이 무게를 들고 있는 겁니다.
A muscle can only work from where the bone is.

Why we start with the spine
척추부터 보는 이유
That's why form cues like "chest up" or "shoulders back" rarely last on their own. If the joints of the upper back don't move, you can hold a good position through one set, and the moment you stop concentrating, the upper back drops back into its rounded shape and pulls the shoulder blade with it. After a few hundred reps like that, the body learns the rounded version as the normal way to lift an arm.
그래서 "가슴 펴세요", "어깨 뒤로" 같은 자세 큐만으로는 오래 못 갑니다. 등 윗부분 관절이 안 움직이면, 한 세트 동안은 좋은 자세를 버틸 수 있어도 신경을 놓는 순간 등이 다시 굽고 날개뼈도 같이 끌려가요. 그렇게 수백 번 반복하면 몸은 굽은 채로 팔을 드는 걸 정상으로 외워버립니다.
So we work in order. First we bring the pain down, because nothing new holds while it's loud. Then we get the joints of the upper back and around the shoulder moving, so the shoulder blade can rest where it belongs, and build strength from that position. The last part is keeping the upper back moving so the shoulder blade stays there.
그래서 순서대로 갑니다. 먼저 통증을 내립니다. 아픈 동안엔 새로 익힌 게 남지 않으니까요. 그다음 등 윗부분과 어깨 주변 관절이 움직이게 해서 날개뼈가 제자리에 놓이게 하고, 바로 그 자리에서 힘을 붙입니다. 마지막은 등이 계속 움직이게 해서 날개뼈가 그 자리를 지키게 하는 일이고요.

Let your arms hang at your sides and look down at them. In most people who come to us with neck and shoulder pain from lifting, the backs of the hands face forward and the inside creases of the elbows face the body. Years at a keyboard and behind the wheel have turned the upper arm inward.
팔을 옆에 편하게 늘어뜨리고 내려다보세요. 운동하다 목과 어깨 통증으로 오시는 분들은 대부분 손등이 앞을, 팔꿈치 안쪽 접히는 자리가 몸 쪽을 보고 있습니다. 키보드와 운전대 앞에서 보낸 세월만큼 위팔이 안으로 돌아가 있는 거예요.
Turn your upper arms gently outward until the elbow creases face straight ahead. The turn comes from the upper arm, not the hand. Most people feel their shoulders drop away from their ears and their elbows settle in toward their ribs on their own. Stop there. The goal isn't to turn out as far as you can. It's to bring the upper arm back from turned-in to neutral, where it was meant to hang. Hold that lightly through the set, and if your shoulders start creeping toward your ears, lower the weight.
팔꿈치 안쪽이 정면을 볼 때까지 위팔을 바깥으로 살짝 돌려 주세요. 손목이 아니라 위팔이 돌아가야 합니다. 그러면 대부분 어깨가 귀에서 스르르 내려가고 팔꿈치가 옆구리 쪽으로 붙는 게 저절로 느껴져요. 거기서 멈추세요. 바깥으로 최대한 돌리라는 게 아닙니다. 안으로 돌아가 있던 위팔을 원래 있어야 할 중립으로 되돌리는 것뿐이에요. 그 상태를 가볍게 유지하면서 운동하시고, 어깨가 귀 쪽으로 올라오기 시작하면 무게를 내리세요.
So how do I hold a barbell?
그럼 바벨은 어떻게 잡아요?
Turning your palms down to grip a barbell isn't a wrist movement. It happens in the forearm. The radius, on the thumb side, rolls over the ulna while the upper arm stays still, the way your forearm turns a screwdriver. When the forearm has lost that roll, the body cheats by turning the whole upper arm inward instead. The palm ends up facing down either way, so it looks right. But the upper arm is turned in again, the same position the pain started from.
바를 잡으려고 손바닥을 뒤집는 건 손목이 하는 일이 아닙니다. 전완에서 일어나요. 엄지 쪽 radius가 새끼손가락 쪽 ulna 위를 굴러 넘어가고, 위팔은 가만히 있죠. 드라이버를 돌릴 때처럼요. 전완이 이 굴림을 잊어버리면 몸은 위팔을 통째로 안으로 돌려서 대신 뒤집습니다. 손바닥은 어느 쪽이든 아래를 보니까 맞아 보여요. 그런데 위팔은 다시 안으로 돌아가 있습니다. 통증이 시작된 바로 그 자세로요.
So set the upper arm first, elbow crease facing forward, then turn only the forearm to take the bar. If the elbow crease turns toward your body as you grip, your upper arm has taken over the forearm's job. With dumbbells, start with the palms facing each other and work toward palms down as the forearm roll comes back. Most people find the wrist sits straighter and the grip feels firmer.
그러니 위팔을 먼저 세우세요. 팔꿈치 안쪽이 정면을 보게요. 그다음 전완만 돌려 바를 잡습니다. 잡는 순간 팔꿈치 안쪽이 몸 쪽으로 따라 돌아가면, 전완이 할 일을 위팔이 가로챈 거예요. 덤벨이라면 손바닥이 서로 마주 보게 잡는 데서 시작해서, 전완의 굴림이 돌아오는 만큼 조금씩 손바닥을 아래로 가져가세요. 대부분 손목이 곧게 서고 쥐는 힘이 단단해지는 걸 느낍니다.

Q. Why does my shoulder still hurt when my exercise form looks correct?
A. A mirror shows the path of your arm, not where your shoulder blade sits. Every rotator cuff muscle starts on the shoulder blade, so if it slides or tips forward, those muscles are pulling from an unsteady base, even when the arm looks right.
Q. Should I squeeze my shoulder blades together when I lift?
A. That cue recruits the rhomboids and levator scapulae, which pull the shoulder blade back and up toward the neck. For many people it adds to the tension at the top of the shoulder instead of relieving it.
Q. Should I turn my arms outward as far as I can?
A. No. Most people with neck and shoulder pain from lifting already carry their upper arms turned inward. The outward turn only brings the upper arm back to neutral, with the elbow crease facing forward. Stop there.
Q. How should I grip a barbell without straining my shoulder?
A. Set the upper arm first, with the elbow crease facing forward, then turn only the forearm to take the bar. If the elbow crease swings toward your body as you grip, the upper arm is doing the forearm's job.
