Here's a sentence we hear in the room all the time. "It doesn't really hurt. It just gets tired."
진료실에서 자주 듣는 말이 있습니다. "아프진 않아요. 그냥 빨리 지쳐요."
A grocery run, three quarters of the way through, and a hand goes onto the cart. Nothing hurts. The right leg has gone heavy, so the cart takes some of it. That had been going on for months and had never been called anything.
장을 보다가 매장 끝쯤에서 손이 카트로 갑니다. 아픈 데는 없어요. 오른쪽 다리가 무거워지니까 카트에 좀 기대는 거죠. 그렇게 몇 달을 지냈는데, 그걸 증상이라고 생각해 본 적은 없었고요.
Or a route run four mornings a week, where the last mile has started costing more on one side than the other. No soreness afterward. That leg just empties earlier.
일주일에 나흘씩 같은 코스를 뛰는 분도 비슷합니다. 어느 날부터 마지막 한 바퀴가 한쪽만 유독 힘들어졌다고요. 끝나고 아프지도 않습니다. 그 다리만 먼저 비어버릴 뿐입니다.
Two different bodies, the same sentence. One side is spending more than the other to do the same work, and it runs out first.
몸도 다르고 하는 일도 다른데, 나오는 말은 같습니다. 한쪽이 같은 일을 하면서 힘을 더 쓰고 있다는 얘기고, 그래서 먼저 바닥나는 거죠.
The hip turns, and then stays turned
고관절은 돌아간 자리에서 멈춥니다
The hip is a ball seated deep in a socket, wrapped in ligament and muscle. Getting it out of there takes a car accident. What we actually see is quieter. The whole bone rotates on its own long axis, a little inward or a little outward, and then stops coming back.
고관절은 공이 컵 안에 깊숙이 들어앉은 구조입니다. 인대와 근육이 감싸고 있어서, 여기서 빠져나오려면 교통사고쯤은 나야 하고요. 진료실에서 보는 건 훨씬 조용합니다. 허벅지뼈가 제자리에서 안쪽이나 바깥쪽으로 조금 돌아간 뒤, 거기서 안 돌아옵니다.
A joint held in rotation makes everything around it work at a length and an angle it was not built for. The muscle still fires. It just costs more. Walk the same distance on both legs and one of them is empty first.
관절이 돌아간 채로 굳어 있으면, 그 둘레 근육은 원래 안 쓰던 길이와 각도에서 일하게 됩니다. 힘은 들어가죠. 다만 같은 일을 해도 더 듭니다. 그러니 같은 거리를 걸어도 한쪽이 먼저 바닥납니다.
The feet will lie to you
발끝은 거짓말을 합니다
People come in having noticed one foot turned out while they were lying in bed, and that is a fair thing to notice. It is also the flimsiest evidence we have. Foot position gets set by the twist of the shin bone, the shape of the foot, muscle tone, and whatever posture someone has been holding for twenty years.
누웠을 때 한쪽 발끝만 벌어져 있는 걸 보고 오시는 분이 많습니다. 눈에 잘 띄니 그럴 만하죠. 그런데 이게 제일 눈에 띄면서 근거로는 제일 약합니다. 발끝 방향은 정강이뼈가 얼마나 비틀렸는지, 발 모양이 어떤지, 근육이 얼마나 당겨져 있는지, 거기에 이십 년쯤 굳어버린 자세까지 같이 만들거든요.
So we leave the feet alone. We lie you down, fold the hip and the knee to a right angle, and lay the shin over to one side and then the other. In that position the shin becomes the hand of a clock. One way, the foot should travel past the edge of the table. The other way, it should come across your body and reach the pocket of the opposite leg. Whichever direction comes up short is the direction that is stuck.
그래서 발끝은 안 봅니다. 눕히고, 고관절과 무릎을 직각으로 접은 다음, 정강이를 한쪽으로 눕혔다가 반대쪽으로 눕혀 봅니다. 이 자세에서는 정강이가 시곗바늘이 되거든요. 한쪽으로는 발이 침대 가장자리를 넘어가야 하고, 반대쪽으로는 몸을 가로질러 반대편 다리 주머니 높이까지 와야 하죠. 둘 중에 모자란 쪽, 거기가 굳은 방향입니다.
Where it actually hurts
실제로 아픈 자리
Put your hand on the side of your hip and you will find a hard bump under the palm. That bump belongs to the thigh bone and turns with it. So a sore spot on the front of that bump and a sore spot on the back of it mean two different things. Checking takes a few seconds. Almost everything after it depends on the answer.
손바닥으로 엉덩이 옆을 짚어 보면 단단하게 튀어나온 뼈가 만져집니다. 이건 허벅지뼈에 붙어 있어서 허벅지뼈가 돌면 같이 돌아요. 그래서 눌렀을 때 앞이 아픈 것과 뒤가 아픈 건 전혀 다른 얘깁니다. 확인하는 데 몇 초 안 걸리는데, 그 뒤가 전부 여기서 갈립니다.
Two other things live in the same neighborhood and get mistaken for this one. Pain that runs as a line down the outside of the thigh to the knee comes from the long band on the outside of the leg. Pressure deep in the buttock that sends a thread of sensation down the leg is a nerve being crowded. We separate those out before the hip gets adjusted, because the correction that helps one of them will make the others louder.
이 근처에 비슷하게 생긴 게 둘 더 있습니다. 허벅지 바깥면을 따라 무릎까지 선처럼 내려가는 통증은 바깥에 길게 붙은 띠에서 옵니다. 엉덩이 깊은 데를 눌렀을 때 다리로 가늘게 뻗어 내려가는 느낌이면 신경이 눌린 거고요. 교정에 들어가기 전에 이 둘을 먼저 걸러냅니다. 하나에 맞는 교정이 나머지 둘은 더 시끄럽게 만들거든요.
A tilted road and a worn shoe
기울어진 길, 한쪽만 닳은 신발
The obvious cause is a fall or a collision, and those come through the door. What fills the schedule is the slow kind. Roads are built with a slight tilt so the water runs off. Run the same route in the same direction long enough and one leg has been landing downhill the whole time. If one shoe is wearing down faster than the other, you are looking at the same thing from underneath.
크게 부딪혀서 생기는 경우가 가장 눈에 띄죠. 그런 분들도 오십니다. 그런데 진료실을 채우는 건 천천히 쌓인 쪽입니다. 도로는 물이 빠지라고 가장자리를 살짝 기울여 놓죠. 같은 코스를 같은 방향으로 오래 달리면, 한쪽 다리는 내내 내리막을 밟아온 셈입니다. 한쪽 신발만 먼저 닳아 있다면, 같은 걸 아래에서 보고 계신 거고요.
Why the low back keeps coming back
허리만 봐서는 잘 안 잡히는 이유
There is a muscle that starts on the bones of your low back and runs down to the inside of the thigh. Turn the thigh bone and that muscle changes length. The far end of it is bolted to your spine. A hip that has rotated and a low back that keeps tightening on one side are often the same problem, seen from above and from below.
허리뼈에서 시작해 허벅지 안쪽까지 내려오는 근육이 하나 있습니다. 허벅지뼈가 돌면 이 근육 길이가 달라지죠. 반대쪽 끝은 허리뼈에 붙어 있고요. 돌아간 고관절과, 한쪽만 자꾸 뭉치는 허리. 같은 문제를 위에서 보고 아래에서 본 것일 때가 많습니다.

Somewhere in the mid-thirties a different picture starts showing up in people who have trained for years. Nothing is tender. Strength tests come back clean. The hip simply does not travel as far as the other one. It answers to the joint being taken through its own axis in many directions, the way you would run a spoon around the inside of a cup.
서른다섯 언저리부터, 운동을 오래 해온 분들에게서 다른 그림이 보입니다. 눌러도 안 아파요. 힘도 멀쩡하고요. 그냥 반대쪽만큼 안 돌아갑니다. 축을 따라 여러 방향으로 열어 줘야 달라지고요. 커피잔 안쪽을 숟가락으로 빙 둘러 훑듯이.
A joint feeds itself by moving
관절은 움직일 때만 영양을 받습니다
The head of the thigh bone is almost entirely covered in cartilage. Blood does not soak into it the way it soaks into muscle. It arrives along the neck of the bone, in vessels that climb the outside of it, and that is the whole supply. The cartilage itself has no blood supply at all. It lives on the fluid inside the joint, and that fluid only moves when the joint moves. Load it, unload it, load it again. That is the pump.
허벅지뼈의 머리는 거의 전부가 연골로 덮여 있습니다. 근육처럼 피가 배어드는 구조가 아니에요. 뼈의 목을 타고 올라오는 혈관 몇 가닥, 그게 공급의 전부입니다. 연골에는 혈관이 아예 없고요. 관절 안을 채운 액체가 먹여 살리는데, 그 액체는 관절이 움직일 때만 돕니다. 눌렸다 풀렸다, 다시 눌렸다. 그게 펌프예요.
So a joint that has stopped travelling through part of its range is not being fed across that part. The rotation we keep measuring tells us how far the pump still reaches. It is also why the textbook has us test the quadriceps. Its top branch runs from L-2, and that level is tied to the blood going into the socket.
그러니 한동안 안 쓴 구간은 그만큼 못 먹은 셈이 되고요. 회전 각도를 자꾸 재는 건 이 펌프가 어디까지 닿는지 보려는 겁니다. 원서가 대퇴사두근 근력을 굳이 재라고 한 이유도 여기 있어요. 맨 위 갈래가 L-2에서 오는데, 이 높이가 관절 소켓으로 들어가는 혈류와 묶여 있다고 보거든요.
And it is why the kind of exercise matters more than the amount. A body can train hard for twenty years and still never take the hip to the end of its rotation. The same flat route, the same direction, the same stride. The joint repeats one small arc ten thousand times and never visits the rest.
운동의 양보다 종류가 중요한 이유도 같습니다. 이십 년을 열심히 해도 고관절을 끝까지 돌려 본 적은 없을 수 있어요. 같은 평지, 같은 방향, 같은 보폭. 고관절은 같은 자리만 만 번 오가고, 나머지는 손도 안 댑니다.
So the movement we look for is the kind that carries the joint all the way around without pounding it. Ground that changes underfoot. Water. A bike with the seat set so the hip actually closes and opens.
그래서 찾는 움직임은 관절을 끝까지 돌려 주되 두들기지 않는 쪽입니다. 발밑이 바뀌는 길, 물속, 안장 높이를 제대로 잡은 자전거. 얼마나 세게가 아니라, 어디까지 도느냐입니다.
We are not chasing a number on a chart. We want that part of the range back in use.
숫자를 늘리자는 게 아닙니다. 안 쓰던 구간을 다시 쓰게 하자는 거죠.
We start by getting the pain down, because nothing underneath it can be worked on while it is loud. Then we go after the soft tissue and the movement pattern that let the rotation settle in. That is the part most people have never gotten to. After that, the work is keeping what came back.
먼저 통증을 내립니다. 아픈 동안에는 그 아래를 건드릴 수가 없거든요. 그다음이 진짜죠. 회전이 눌러앉게 만든 연부조직과 움직임의 패턴으로 들어갑니다. 여기까지 와보신 분은 드뭅니다. 그 뒤로는 되찾은 걸 지키는 일이 남고요.
If one leg has been going empty before the other, or one of your shoes is wearing down faster, that is worth an examination.
한쪽 다리만 먼저 힘이 빠지거나, 신발 한 짝이 유독 빨리 닳고 있다면 한번 검사해 볼 만합니다.
Wear the shoes you actually walk in, and come in something you can move in.
